We accept most insurance plans including Tri-Care, Medicare, Medicaid and Denali Kid Care. We also offer a sliding fee discount for qualified patients and families.

Health Insurance Marketplace

You can now enroll in health coverage through the Health Insurance Marketplace at HealthCare.gov during Open Enrollment Periods.

Through the Marketplace you can apply, compare your options, and find out if you can get free or low-cost coverage. When you’re ready to enroll, you’ll decide how you pay your premiums or get help signing up for Medicaid if you’re eligible.

Some individuals may qualify for Special Enrollment Periods outside of Open Enrollment if they experience certain events including loss of other health coverage. Please contact us at 907-822-3203 for more information or to make an appointment with one of our Certified Counselors.

Cross Road Health Ministries, Inc. is a Certified Application Counselor Organization with Certified Applicant Counselors who can assist with the application process during the Open Enrollment and Special Enrollment Periods.

  • You are responsible for charges not covered by your insurance.
  • Lab and x-ray services may be billed separately.
  • You may qualify for our sliding fee discount.

Sliding Fee Discount

Cross Road Health Ministries, Inc. offers a sliding fee discount for low income patients (individuals and families) who qualify.

This program is based on Federal Poverty Guidelines. If qualified, this discount would help with the prices of office visits and prescriptions (at our Glennallen pharmacy) and other ancillary items.

Applications may be picked up at your clinic. Income verification is needed to complete the application process.

FAQ about Sliding Fee Discount

Anyone can apply who is seeking services.

Persons with income of less than 200% of Federal Poverty Guidelines, which considers household size and all sources of income before taxes.

Persons whose income exceeds 200% of the Federal Poverty Guidelines or who do not bring in required proof of household income.

Complete the Cross Road Sliding Discount Application form and bring required proof of household income and family size. Please call Cross Road at (907) 822-3203 if you have any questions. Examples of proof include:

  • A copy of last year’s income tax return.
  • Three months of paycheck stubs showing gross income before taxes.
  • A letter from your employer on company letterhead stating your gross monthly income.
  • Social Security or unemployment check stubs.
  • Three months of bank statements showing direct deposits of employment earnings, social security, unemployment insurance, etc.

You are eligible for the sliding fee discount for one year after you are approved.
